      • Opportunity: Nurse-led medication monitoring and adverse events
        • Patient safety is a recognised priority for healthcare organisations and professionals, but there are underlying weaknesses in current medication practice, particularly medication monitoring for known adverse effects of prescribed drugs. Reviews indicate that 4-6% of hospital admissions are due to adverse drug reactions, many of which are preventable. Failure to monitor for common problems is responsible for the majority of adverse drug reactions. The need for further research into the relative risks and benefits of treatments, and improved systems for actively monitoring patients for the known adverse effects of prescribed medications is well documented.
        • The aim of this project is to develop current medication monitoring practices for detecting and actioning adverse drug reactions. It will specifically examine the possibilities of nurse-led medication monitoring.
        • If you are interested in getting involved or would like more information, please contact Anna Coote, telephone no. 029 2043 1774, by 25th November 09 and provide any details or specific information about yourself that you think may be helpful.

In order to help, CHRE would like to hear about your experiences with the regulators. You can provide written feedback via their website or email. Please be aware that any information submitted will be shared with the relevant regulator and is disclosable under the Freedom of Information Act 2000.
